ABSTRACT

BACKGROUND: Birth asphyxia is the main cause of neonatal mortality and morbidity worldwide. Some studies indicate intimate partner violence during pregnancy is a risk factor for birth asphyxia. In Ethiopia, intimate partner violence during pregnancy is reported to be high. Despite this high prevalence, there is a lack of data about the association of birth asphyxia and intimate partner violence. The aim of this study was to assess the prevalence of intimate partner violence during pregnancy and its associated factors with birth asphyxia in health facilities in the Tigray region of northern Ethiopia. METHODS: This was an institutional-based cross-sectional study conducted at select health facilities in the Tigray region of Ethiopia. Random sampling technique was employed to select health facilities and systematic sampling was used to select 648 study participants. Data was entered by using Epi info version 3.5.1 and was analyzed using SPSS version 20. Bivariate and multivariate analysis was done to assess the association between exposure to intimate partner violence during pregnancy and birth asphyxia after adjusting for possible confounders. RESULTS: The prevalence of intimate partner violence during pregnancy was 47(7.3%). Eighty two (12.7%) babies were delivered with birth asphyxia. Intimate partner violence during pregnancy had a significant association with birth asphyxia, AOR (95% CI) = 4.4(2-9.8). In addition to this, other factors that were associated with birth asphyxia include place of residence [ AOR (95% CI) = 2.7(1.55-4.8)], age > 19 [AOR (95% CI) = 2.9(1.29-6.5)], age 20-35 [AOR (95% CI) = 3.1(1.06-9.3)], gestational age < 37 weeks [AOR(95% CI) = 7.2(3.5-14.8)] and low birth weight [AOR(95% CI) = 3.9(2.1-7.3)]. CONCLUSIONS: The prevalence of birth asphyxia in this study is high and is further increased by intimate partner violence during pregnancy. Health care providers and policy makers should take measures aimed at preventing intimate partner violence during pregnancy to reduce harm to the mother and adverse birth outcomes.

ABSTRACT

Apart from the inequality in vaccination, war zones and areas where communication is disrupted are affected by myths and misconceptions about COVID-19 vaccines, heightening vaccine hesitancy. Local data on adverse events of the vaccines and their mildness can increase confidence and acceptance of the vaccines in the respective population. In areas of conflict and communication blackouts, the perception of the vaccines by health workers is of paramount importance as public health recommendations may not reach the public. Therefore, the scientific evaluation of adverse events following COVID-19 vaccination in such areas is invaluable. This cross-sectional, facility-based study was conducted using a structured, interviewer-administered questionnaire to assess the adverse events experienced by healthcare workers who received the Janssen COVID-19 vaccine. The sample was divided proportionally to the number of vaccinated healthcare workers for the different healthcare professions, and participants were then randomly selected from each profession. Prior to data collection, a pilot test was conducted with 5% of the sample size outside the selected hospital. The study was conducted using a structured questionnaire completed by an interviewer to assess adverse events in 442 healthcare workers who had received the Janssen COVID-19 vaccine between July 11 and 25, 2022. The study period was from August 15 to September 15, 2022. A significant number of healthcare workers [366 (83.3%); 95% CI 79.5%, 86.5%] experienced at least one adverse event. Nearly 90% of participants reported that the adverse events were mild to moderate. Pain at the injection site [307 (69.5%); 95% CI 65.0%, 73.6%] and headache [247 (55.9%); 95% CI 51.2%, 60.4%] were the most common local and systemic adverse events, respectively. Two HCWs experienced anaphylactic reaction. Younger age was significantly associated with the occurrence of adverse events. We deciphered that the adverse events reported by the study participants were not different from the typically occurring vaccine-related adverse reactions, and therefore concluded that post-vaccination reactions in healthcare workers were minor. Although vaccination in Tigray is currently stalled due to the siege, responsible stakeholders should develop a mechanism to track population-wide adverse events once the vaccines start to rollout.

ABSTRACT

Intimate partner Violence (IPV) can affect any woman, irrespective of their economic status, religion, or culture. This is a human-rights issue and due to its prevalence and adverse effects on pregnancy and birth, it must be given greater attention. Further, there is a lack of data in the Tigray region about adverse birth outcomes due to intimate partner violence during pregnancy. The aim of this study was to assess intimate partner violence during pregnancy and its association with low birth weight and preterm birth in Tigray region. Across-sectional study design was used. 647 women were involved in the study. Simple random sampling techniques were employed to select health facilities and systematic sampling was used to select study participants. Data were entered using Epi info version 3.5.1 and was analyzed using SPSSversion 20. Logistic regression analysis was conducted to assess the association between exposure to intimate partner violence during pregnancy and preterm birth and low birth weight while adjusting for possible confounders. The prevalence of intimate partner violence during pregnancy was 7.3% and the prevalence of low birth weight and preterm birth were 18.5% and 10.8% respectively. There was a statistically significant association between exposure to intimate partner violence during pregnancy and low birth weight. After adjustment for socioeconomic status, women's habits and obstetric factors, the pregnant women who were exposed to intimate partner violence during pregnancy were two times more likely to have a child with a low birth weight (2.39 (95% CI: 1.26-4.55)). The prevalence of intimate partner violence during pregnancy, low birth weight, and preterm birth in this study was high. Women who experienced intimate partner violence during pregnancy had an increased risk of low birth weight. These findings justify a call to the federal minster of health to take measures aimed at avoiding intimate partner violence during pregnancy to reduce adverse birth outcomes.

ABSTRACT

The war and siege in Tigray led millions to displace internally. More than three-fourths of the health facilities were either destroyed or not functional as the equipment and other resources were stolen. Furthermore, the remaining functioning health facilities were flooded beyond their capacity, resulting in many patients received treatment late, and ending with complications including life loss. Mekelle City is one of the largest towns with many internally displaced people from different areas of Tigray. To provide services for the most vulnerable populations, 11 IDP clinics were opened for internally displaced people and the surrounding host community in Mekelle. A total of 6732 patients received clinical services, of which 3465 were males. The age of the patients was in ranged of 24 days to 95 years. A total of 364 patients were emergency cases and 428 outbreaks were seen. A total of 722 patients with chronic illnesses received follow-up services, the most common being hypertension (112), diabetes (79), and asthma (70). Overall, 1198 investigations were done and 1339 were referred to higher-level healthcare facilities. Upper respiratory infection (n = 976), acute gastroenteritis (n = 667), and pneumonia (n = 612) were the most common disease conditions in IDP clinics. Antibiotics were the most commonly prescribed medication for 2468 patients, followed by anti-pain/pyretic (1402). This community engagement showed us that, it is possible to continue healthcare services when health facilities get collapsed during crisis owing to the relocation and mobilization of available resources.

BACKGROUND: Trachoma is the most common infectious cause of blindness in the globe. Trichiasis surgery is the best treatment option for this disease. Despite efforts done to eliminate blinding trachoma, there is limited evidence on the surgical uptake of trachoma trichiasis in Ethiopia. This study was aimed to assess the uptake of trachoma trichiasis surgery in Southern Tigray, Ethiopia. METHODS: Mixed cross-sectional study was employed among 409 participants. Study participants were selected using a consecutive sampling technique. Pretested and interviewer-administered data were collected using a structured questionnaire. Binary and multivariable logistic regression was done to identify associated factors. Adjusted odds ratios 95% CI was estimated to show the strength and direction. Variables with p-values <0.05 were considered statistically significant. For qualitative data, 4 focus group discussions were conducted with 40 participants and described by thematic analysis then triangulated with quantitative findings. RESULTS: About 234 (57.9%, 95% CI: (53.2, 62.9)) participants utilized trachoma trichiasis surgery (TT). History of trachoma trichiasis (TT) for >2 years [AOR: 0.4, 95% CI: (0.22, 0.72)], informed about surgery program by health workers [AOR: 0.3, 95% CI: (0.13, 0.71)], history of TT surgery [AOR: 0.18, 95% CI: (0.05, 0.6)], absence of someone to care the family [AOR: 14, 95% CI: (6.9, 28.6)], companion [AOR: 8.9, 95% CI: (4.3, 18.3)], nearby health facility [AOR: 2.4, 95% CI: (1.1, 5.4)], work load [AOR: 8.8, 95% CI: (4.6, 17)], fear [AOR: 4.3, 95% CI: (1.8, 10)], and believing eye drop can treat TT [AOR: 3.9, 95% CI: (1.4, 11)] were significantly associated factors. CONCLUSION: More than half of the participants accepted the TT surgical uptake. Strengthening community awareness on proper eye care, and effective treatment options, and addressing the negative attitude towards surgical treatment in the community are important measures to achieve the elimination target of trachoma.

ABSTRACT

OBJECTIVE: To describe the clinical features and assess the determinants of severity and in-hospital mortality of patients with coronavirus disease 2019 (COVID-19) from a unique setting in Ethiopia. METHODS: Consecutive patients admitted to a COVID-19 isolation and treatment centre were included in this study. The overall clinical spectrum of COVID-19, and factors associated with risk of severe COVID-19 and in-hospital mortality were analysed. RESULTS: Of 2617 quarantined patients, three-quarters (n = 1935, 74%) were asymptomatic and only 114 (4.4%) presented with severe COVID-19. Common characteristics among the 682 symptomatic patients were cough (n = 354, 50.6%), myalgia (n = 212, 31.1%), headache (n = 196, 28.7%), fever (n = 161, 23.6%), dyspnoea (n = 111, 16.3%), anosmia and/or dysgeusia (n = 90, 13.2%), sore throat (n = 87, 12.8%) and chest pain (n = 77, 11.3%). Factors associated with severe COVID-19 were older age [adjusted relative risk (aRR) 1.78, 95% confidence interval (CI) 1.61-1.97; P < 0.0001], diabetes (aRR 2.00, 95% CI 1.20-3.32; P = 0.007), cardiovascular disease (aRR 2.53, 95% CI 1.53-4.17; P < 0.0001), malignancy (aRR 4.57, 95% CI 1.62-12.87; P = 0.004), surgery/trauma (aRR 23.98, 95% CI 10.35-55.57; P < 0.0001) and human immunodeficiency virus infection (aRR 4.24, 95% CI 1.55-11.61; P = 005). Factors associated with risk of in-hospital mortality included older age (aRR 2.37, 95% CI 1.90-2.95; P < 0.001), malignancy (aRR 6.73, 95% CI 1.50-30.16; P = 0.013) and surgery/trauma (aRR 59.52, 95% CI 12.90-274.68; P < 0.0001). CONCLUSIONS: A significant proportion of cases of COVID-19 were asymptomatic, and key comorbid conditions increased the risk of severe COVID-19 and in-hospital mortality. These findings could help in the design of appropriate management strategies for patients.

BACKGROUND: Globally malaria affects 212 million people and causes 438,000 deaths each year. Ensuring early and timely treatment of malaria is important for preventing and controlling of life-threatening complications and further transmission. Even though malaria treatment is widely available in Ethiopia, 47-84% of patients present after 24 h of onset of first symptoms. This study assessed the determinants of delay for malaria treatment in Tigray, Ethiopia. METHODS: A health facility-based case-control study design in northwestern zone of Tigray was conducted from September 2018 to January 2019. All the study participants enrolled were confirmed malaria patients (by microscopy or rapid diagnostic test) and who sought treatment. Cases were defined as malaria patients who sought treatment after 24 h of the onset of the first symptom and control were those who sought treatment within 24 h onset of symptom. A structured questionnaire was used to collect data on the determinants of malaria treatment delay. Data were entered into EpiInfo 7.0 and exported to SPSS 20.0 for analysis. Binary logistic regression was computed to identify predictors of delay for malaria treatment. RESULTS: In total 161 cases and 161 controls were identified. Being residents of Tahtay Adyabo district (AOR = 2.84, 95% CI 1.29-6.27), having no formal education (AOR = 2.39, 95% CI 1.09-5.22), the decisions to seek health care being taken by the patient (AOR = 2.38 95% CI 1.09-5.2), the decisions to seek health care being taken by their fathers (AOR = 2.52, 95% CI 1.13-5.62), and having good knowledge about malaria symptoms (AOR = 2.02, 95% CI 1.21-3.39) were found determinants of delay for malaria treatment. CONCLUSION: In this study, delays in obtaining treatment for malaria were associated with having no formal education, knowing about the signs and symptoms of malaria, living in Tahtay Adyabo district, and decision-making on seeking malaria treatment. The results suggests having treatment commenced at sites closer to the community and strengthened awareness-raising activity about the importance of early seeking for all with malaria-like symptoms, especially for household heads would contribute to improved treatment and reduced complications from malaria.

BACKGROUND: Ethiopia had an increasing trend of morbidity and mortality due to road traffic injury. Road traffic injured patient's recovery rate is affected by many different factors. Those factors might affect the duration of time to recovery. Therefore studying the median time to recovery and its predictors of road traffic injured patients will be needed to act upon the patient's hospital provided service. METHOD: A retrospective cohort study design was employed. The study population was all admitted road traffic injured patients in Ayder tertiary hospital. We have used the total of all three-year RTI patients' chart from 2015 to 2017 found in the hospital. After excluding incomplete charts for major variables the sample size was 322. Descriptive statistics, life table, Kaplan-Meier, log-rank test and assumptions of the Cox proportional hazard model was applied. Bi and multivariate Cox regression analysis, hazard ratios and associated 95% CI were estimated. RESULT: Male to female RTI patient ratio was 3:1. Of the total 258(80.1%) had been recovered and the median survival time to recovery was 15 days (interquartile range 7-29). From those recovered, 104(40.3%) had been referred from other health facilities. Availability of referral form linkage [adjusted hazard ratio = 1.5, CI (1.1-1.9)], mild and moderate glass coma scale [adjusted hazard ratio = 2.3, CI (1.3-3.9)], conservative management [adjusted hazard ratio = 1.6, CI (1.2-2.1)], and not having organ injury [adjusted hazard ratio = 1.6, CI (1.1-2.3)] were associated with time to recovery in multivariate analysis. CONCLUSION: Median time to recovery of road traffic injured patients was relatively good. Being referred from another health facility, mild and moderate glass coma scale, conservative management and without organ injury was positively associated with time to recovery of road traffic injured patients. We would like to recommend for future prospective studies to determine the time to return to work of road traffic injured patients and quality of life after the injury.
